Founded in 1882, Montefiore boasts a 133-year legacy of providing compassionate, supportive and high quality care to seniors and others in need. Our mission is to provide a comprehensive system of support services related to healthcare needs throughout the aging process. Guided by Jewish values, Montefiore is dedicated to a standard of excellence and personalized care for all members of the greater Cleveland community, enabling individuals to live with dignity and security and as independently as they are able.

Montefiore has two campuses, one in Beachwood and the other in Chagrin Falls, Ohio that continue to evolve and expand to reflect changing community needs. Montefiore’s Beachwood campus offers a full array of healthcare services including post-hospital care, long-term care, hospice and palliative care, and assisted living memory care. The Weils campus in Chagrin Falls offers  assisted living services for an active senior living community since 2002. In addition to assisted living, The Weils campus offers a full scope of healthcare services to seniors, including a rehab therapy pavilion and an assisted living memory care community opening in spring 2015.

 

Beyond on-campus services, Montefiore offers a full line of home healthcare services including private duty, home hospice and palliative care, therapy services and activities of daily living support. Based on our organizational strategic plan and initiatives, while recognizing the growing community need for comprehensive, innovative dementia care services, Montefiore continues to develop service offerings such as the In-Home Memory Care Support Program, which will benefit those with dementia and Alzheimer’s, their families and the community as a whole throughout the continuum of care.
